REGISTRATION FOR SPRING SPORTS 2014

Registration for Spring Sports for Daniel Hand High School is now open. Forms may be dropped off in the Health Office or the Athletics Office between 8:15 AM and 12:00 PM. The final day to accept registration forms/fee is Friday, March 7, 2014 at 12:00 PM.. This is an important deadline because time is needed before the start of the season to review forms, compile medical information, record fees and inform coaches of players’ eligibility. Please help us help your children to participate in sports without any delay.

In order to participate in ANY sport offered by the Madison Public Schools a student must have the following forms on file with the Health Office:

• Health Assessment Record
• Code of Conduct
• Registration Form/Consent
• Fee (or Confidential Waiver Form).

All forms are available on the website – www.danielhand.org - and in the Health and Athletics Offices.
