To: The Parents of Current 11th Grade Students at DHHS RE: State Mandated 10th Grade Physical Examinations and Immunization Record
Dear Parents/Guardians,
The State of Connecticut mandates Physical Examinations to be updated in the 10th grade to include a physical examination by a health practitioner, an oral examination by a health practitioner or dentist, a parent report of health, and an up to date immunization record. Each student should have had a physical examination between June 1, 2019 and June 1, 2020. The Health Office at DHHS is still missing a few students. If you know that you have not handed in your child's physical during their 10th grade year, please submit it via email to Stephanie Lesnik, RN at lesniks@madison.k12.ct.us or fax it to the Health Office at DHHS (203) 245-6392. If you are not certain as to whether you have submitted your child's physical, please contact Stephanie Lesnik, RN via the above email. The state of Connecticut has not granted any extensions or exceptions to mandated physicals for students. If your child has not had a 10th grade physical to date, please contact your medical provider and arrange one as soon as possible. Medical providers are aware that the state is not granting extensions to this mandate and are therefore willing to provide priority to getting these completed.
