Winter Registration
Dear Students and Families,
Registration for the Winter Athletic Season is now open! Please log in to your FinalForms account and register for your winter sport as soon as possible to ensure you don’t miss any important updates from coaches.
You may only participate in one sport per season, so please make your selection carefully. Coaches will be sending out information and or hosting pre-season meetings in the coming weeks. If you have any questions about a specific program, please reach out to the head coach directly, as they can best address sport-specific details.
Just so you know, we are in the final stages of hiring a coach for Ski and Dance. More information to come shortly. Thank you for your patience.
Important Deadline:
All student-athletes must be registered by Friday, November 21st to ensure eligibility for the first days of practice or tryouts. This deadline allows our office staff, school nurse, and athletic trainer adequate time to verify eligibility and medical clearance. Given the timing of Thanksgiving Break and the start of winter sports, meeting this deadline is essential. Students who register after November 21st may not be cleared in time to participate in the opening days of their season.
If you are still undecided about participating, we encourage you to register anyway—this helps our coaches and department gauge interest and plan accordingly.
Additional Reminders
Physical Examination:
A current physical must be on file with the school nurse. Physicals are valid for 13 months. Please check your FinalForms profile to verify your physical’s expiration date. You can send in to the health office or upload on FinalForms.
Return Fall Sports Equipment:
If you participated in a fall sport, all equipment and uniforms must be returned before participating in winter sports. Failure to do so may result in a “school obligation” hold in your FinalForms account, which will prevent participation.
Impact Testing:
All student-athletes are required to complete an Impact Test every two years. Coaches and our Athletic Trainer will notify students who need to complete or update their test. You can check your FinalForms account for your Impact Testing status.
Athletic Eligibility:
Per CIAC and Daniel Hand High School guidelines, students must be enrolled in and passing at least four classes to be eligible. First-trimester grades will determine winter eligibility, which will be reflected in your FinalForms profile once grades are finalized.

Creating a FinalForms Account
Madison Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service that will further streamline the registration process for athletics and other student activities. We will no longer use FamilyID/ArbiterID. FinalForms allows you to complete and sign all athletic participation forms online. One of the most convenient features is that your information is saved from season to season and year to year, meaning you will never need to enter the same data twice! The platform also pre-populates information wherever possible for each of your children, saving you valuable time.
You can review your information at any time to ensure it is up to date. Parents/guardians are required to sign forms once per year and anytime there is an update.

Polson Middle School will use FinalForms for after-school activities, intramurals, and field trips. Setting up your account now will make the process that much easier once we register to participate.
Important note regarding physicals:
You do not need to upload a physical to FinalForms if you are confident that the Health Office at your child’s school already has a current physical on file. We kindly ask for your patience as we update physical records in the new system during this transition.
