How to Request a Transcript

Official transcripts from Daniel Hand High School or the Hammonasset School are only sent directly to schools, businesses, or agencies—they are not mailed to your home address. Please follow the instructions below to request your transcript.

For Graduates:

1.     Complete the Transcript Request Form.

  • This form is for graduates of Daniel Hand High School or the Hammonasset School.

2.     Submit the Form to Cynthia Kallert.

  • Once completed, email or mail the form to the following address:
