PROCESSING FEE: $3.00 per request CASH or CHECK made payable to DHHS

Official transcripts are only sent to a school/business/agency and are not sent directly to your home. This transcript request form is for graduates of Daniel Hand High School or the Hammonasset School.

Current high school seniors looking to have transcripts sent with college applications, please visit the "Senior Information" page for a link to the blue form or stop by the counseling office.
