2026-2027 School Year Registration Dates
Fall Sports/Activities: Aug 25, 2026 - Sep 12, 2026
Winter Sports/Activities: Nov 3, 2026 - Nov 26, 2026
Spring Sports/Activities: Feb 16, 2027 - Apr 3, 2027
Please note that late registrations may delay eligibility, so even if your child is unsure about participating, we encourage you to register before the deadline.
To Be Eligible to Participate:
1. Register on FinalForms
- Families should have received an email from FinalForms with setup instructions.
- This is our new registration platform, and we appreciate your patience as we transition.
- If you have any questions, don't hesitate to get in touch with the Athletic Office.
2. Submit a Current Physical Examination
- A valid physical must be on file with the school nurse.
- Physicals are valid for 13 months from the date of the exam.
- You can verify your child’s physical status in your FinalForms account.

Creating a FinalForms Account
Madison Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service that will further streamline the registration process for athletics and other student activities. We will no longer use FamilyID/ArbiterID. FinalForms allows you to complete and sign all athletic participation forms online. One of the most convenient features is that your information is saved from season to season and year to year, meaning you will never need to enter the same data twice! The platform also pre-populates information wherever possible for each of your children, saving you valuable time.
You can review your information at any time to ensure it is up to date. Parents/guardians are required to sign forms once per year and anytime there is an update.
To get started:

com/ - Follow the prompts to create your account, PARENT, and sign all required forms.

Polson Middle School will use FinalForms for after-school activities, intramurals, and field trips. Setting up your account now will make the process that much easier once we register to participate.
Important note regarding physicals:
You do not need to upload a physical to FinalForms if you are confident that the Health Office at your child’s school already has a current physical on file. We kindly ask for your patience as we update physical records in the new system during this transition.
