Alternate Transportation Form
If your child requires alternate transportation for day care purposes, please complete the form below. Requests for alternate pick-up and/or drop-off locations must be consistent and within the district of your child's school. We can schedule Mondays only; Monday, Wednesday, Friday, etc.
Once bus routes have been established, alternate transportation requests will not be accommodated. If you are unable to meet the bus, please contact the TCLC to arrange parental pick-up at school.
Every effort will be made to honor your request. To assist in scheduling of buses, please return the form as soon as possible. Please contact your school for a list of day care providers serviced within your school attendance area. Please remember, transportation will only be provided to a day care facility located in your designated school district.
